Time is brain. Stroke care has done a great job at getting that message home. Door-to-needle is also applicable to treating Alzheimer's patients. Delays between cognitive complaint and treatment do not allow patients early benefit. Bringing a patient from a cognitive complaint to infusion is a skill. Master clinicians shorten the timeframe. We think a 90 day goal is appropriate.

Assuming there is an effect in more moderate disease, those with later starts will never catch up to early starts.
